Referral Procedure

Referrals are accepted from schools, physios, occupational therapists or indeed any agency that has identified an issue or difficulty. The standard Pupil Referral form requests the following information:

  • personal and school details,
  • nature of physical disability,
  • areas of concern,
  • statements,
  • relevant reports/IEPs,
  • school/parental permission

Schools are contacted and an initial assessment visit made. The school is then asked to complete a Referral Form which highlights the issues/difficulties to be addressed. Parents are informed of the involvement of the Service by letter.
