Referral Procedure

 

The Outreach Service provided by Penn Hall School is very much based on meeting the needs of individuals in mainstream schools.

 

Therefore, the issues/barriers/difficulties that have resulted in a referral are specific to that particular child, and are affected by the staff, the setting and the overall school context. Consequently, the levels of support and outcomes vary considerably between each individual and school.

Referrals are accepted from schools, physios, occupational therapists or indeed any agency that has identified an issue or difficulty. The standard Pupil Referral form requests the following information:

 

Inclusion with mainstream activites

 

Bullet Point graphic personal and school details,
Bullet Point graphic nature of physical disability,
Bullet Point graphic areas of concern,
Bullet Point graphic statementing,
Bullet Point graphic relevant reports/IEPs,
Bullet Point graphic school/parental permission.


Schools are contacted and an initial assessment visit made. The school is then asked to complete a Referral Form which highlights the issues/difficulties to be addressed. Parents are informed of the involvement of the Service by letter.
